Gather all your ingredients and tools: a heavy-bottomed saucepan, a wooden spoon, a measuring cup, and a square baking dish lined with parchment paper.
In the saucepan, combine the granulated sugar, cocoa powder, and salt. Stir until well mixed.
Pour in the whole milk and add the unsalted butter. Turn the heat to medium and stir continuously to dissolve the sugar and cocoa.
Once heated, allow the mixture to boil undisturbed for about 5 minutes until it reaches a rich, dark color.
Remove from heat and let sit for a minute. If using, fold in the chopped nuts.
Vigorously stir the mixture until it cools slightly and thickens.
Pour the fudge into the prepared baking dish and smooth the top with a spatula.
Allow to cool at room temperature for a few hours or refrigerate until set.
Once set, lift out using the parchment paper and cut into squares.
Store in an airtight container at room temperature or in the refrigerator.
Notes
For variations, consider adding peanut butter, mint extract, or chocolate chips.
